Affiliate Provider Responsibilities and Procedures

Capital EAP will contact an affiliate provider to request EAP services for an employee or eligible family member. Approval for the maximum number of sessions is given at that time. The affiliate provider must then contact the client within 24 hours to offer an appointment. An appointment within 3 to 5 business days must be offered for non-urgent situations. Clients in crisis must be seen within 24 hours. Affiliate providers must make available daytime and evening appointments. Appropriate paperwork must be completed at the first session. Upon completion of an initial assessment, the affiliate provider will determine whether or not a resolution can be reached within the approved number of EAP sessions. If it is reasonable to believe resolution can be reached within the sessions allowed, the affiliate provider will proceed with counseling. If it is determined that resolution is unlikely within the sessions allowed, the affiliate provider will refer the client to the most appropriate clinician participating in the clients health care program (if the affiliate provider is self referring, two other referrals must also be given.) Cases should be closed within two weeks of the last contact with the client. All appropriate paperwork must be completed and returned at the completion of the case and received by Capital EAP within 3 weeks of the final session.
