Our programs are designed to encourage the personal development of children and youth while working to strengthen and support healthy relationships among family members. All youth deserve a chance to discover and put to use their inherent strengths and talents in order to become successful, contributing adults. Our approach is to promote positive, healthy child and adolescent development by fostering responsibility, resiliency, and respect for self and others.
About 13% of all births in the U.S. are to teens. Teenage pregnancy impacts almost every facet of community life including education, employment, homelessness, financial dependency and healthcare.
Collaborative partner Whitney M. Young Jr. Health Care Center provides a full time Case Manager who has expertise in client advocacy. This individual assists teens with obtaining primary health care and related health care services. Also, this individual is responsible for insuring that teen customers are aware of the importance of maintaining regular healthcare monitoring and, if parenting, how critical health care is in the development of their child. |

In addition to the roles of our collaborative partners, Family and Children's Service of the Capital Region contributes a full time Team Leader/Program Coordinator who coordinates the work load and allocation of resources among the three organizations. The Team Leader/Program Coordinator is instrumental in assessing customer and program needs, facilitating lines of communication and planning and determining the direction of the program.
In addition, a Clinical Supervisor provides clinical oversight for each Case Manager. Many teens have mental health issues and need a higher level of clinical intervention than the Case Manager is equipped to handle. The Clinical Supervisor consults with the YES team, attending meetings and providing emergency back up for issues related to suicide, homelessness, domestic violence.
Children as young as nine and up to twenty one are eligible. Services are free and confidential. YES is recognized by the
